 Document Purpose 
COVID COMPLIANCE DOCUMENTSRunner and Volunteer Daily Health CertificationCRT Social Distancing and Health Protocols;   Los Angeles County Health Protocols For Youth Sports     RUNNERS and Volunteers must submit a Health Certification for each day activity in which they participate.  The printed certification is to be shown to the coach or parent volunteer in charge and placed in the special box provided (not passed by hand).  The CRT Social Distancing and Health Protocols shall be in effect at all times, until such time as the COVID requirements placed on Youth Sports Organizations remain in effect.  The Los Angeles County Health Protocols provides greater detail.
Claremont Running Team's  Disclaimer Assumption of Risk and Disclaimer for Youth CRT must have a current Disclaimer, Assumption of Risk, and Waiver on file before Youth Runners or Student Volunteers may participate or Register for Practices & Events including practices.  Guest youth taking advantage of a free practice trial must also have a signed copy on file.

Claremont Running Team's  Disclaimer Assumption of Risk and Disclaimer for Parent Volunteer

CRT must have a current Disclaimer, Assumption of Risk, and Waiver on file for Parent Volunteers.
CRT Drop-off / Pick-up Authorization Form Allow persons other than parent / legal guardian to drop-off, accompany, and pick up YOUTH RUNNERS at/from Practices & Events, including quarterly practices.  Youth who will walk or bike home from practices must have a completed Authorization Form on File.
Claremont Running Team's Youth Insurance Information Form.  CRT requires that Youth Runners and Student Volunteers have their own medical insurance. CRT does not provide medical insurance or coverage for CRT RUNNERS, STUDENT VOLUNTEERS for any CRT PRACTICES & EVENTS.

Claremont Running Team, Articles of Incorporation, 501(c)(3)  Articles of Incorporation, filed with the California Secretary of State
IRS Tax Exemption Letter CRT is tax exempt from federal tax under IRS Section 501(c) (3), and has been determined to be a Public Charity under IRC Section 509(a)(2).  Donors to CRT can deduct their contributions under IRS Section 170.
